I would like to designate this gift to the following Please choose one of the following (Mouse over each option for more information):	Annual Friends of Hospice Annual Tree of Life Blasenstein Memorial Fund for Music Therapy General Fund Memorial Fund Annual Friends of Hospice A donation to the annual Friends of Hospice (FOH) campaign will be applied to the areas of assistance that the board of trustees of the Foundation has determined will receive financial support for the current year. This is an unrestricted gift and typically reflects the donors annual gift to the agency. Donations to FOH provides Hospice with the ability to continue to offer its excellent services to those on the hospice program. Annual Tree of Life A donation to the annual Tree of Life is done so as a way of remembering a loved one during the end-of-the-year Holiday Season. The gift is applied to the focus of the annual campaign. All donors to the Tree of Life may choose to have the name(s) of their loved one(s) placed on a paper ornament and hung on one of the Hospice trees at either The Poughkeepsie Galleria or Hudson Valley malls during December. The gift is unrestricted and is applied to the areas of service deemed at most need as determined by the board of trustees. Memorial Fund Memorial donations may be made any time throughout the year. Most memorial donations are made in memory of patients who have been on the Hospice program. However, individuals may make donations to Hospice in memory of an individual who was not on the Hospice program, as well. Please complete the appropriate section on the donation web page which applies to memorial gifts.
