STATUS:   Full Time (2 Positions)

HOURS:   Monday through Friday, 8:30 am - 4:30 pm.

TEAM:   Ulster and Nursing Home

FUNCTION:   As a member of the Hospice Interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of the patient/family/staff by providing counseling and emotional support to help them through issues of terminal illness, grief and loss.  The social worker provdes concrete social services and actively participates in developing the patient/family care plan.

EDUCATION:   Masters Degree in Social Work.  LMSW upon completion of first year of employment; LCSW preferred.  Certified social worker, licensed to practice in NYS.

EXPERIENCE:   Minimum two years of social work experience with one year in hospice and/or grief counseling, community home health preferred.  Knowledge of family and group treatment.  Dynamics of terminal illness and grief and bereavement.  Excellent counseling, advocacy and collaboration skills.

Valid NYS driver’s license and acceptable driving record.
